To obtain a business license from the Selectboard to operate a business in Petersham as required by Town Bylaws, please complete the Business Application Town Sign-off Form, Town Clerk’s DBA Application Form, MA DoR Revenue Enforcement and Protection Form, and Worker’s Compensation Insurance Affidavit, with required signatures, and return these items to the Selectboard Office.
In the summary of your business description, please include a brief description of your business and what you are asking to be licensed for; your business location, and any parking considerations (see the zoning section of Town Bylaws for parking).
A check payable to the ‘Town of Petersham’ in the amount of $15.00 must be included with your application.
The Town Business License is valid for three years. License renewal is required every three years therafter.
Each of the Town Departments/Boards listed on the Sign-Off Form must sign before a Business License will be issued by the Selectboard. Note: the Historic District Commission sign-off is required only if the business is located in the Historic District. Please see the Historic District Map of Parcels.
A Business License issued by the Selectboard is NOT the same as Permits required by the Town Clerk or the Zoning Board of Appeals (ZBA).
- Town Clerk needs: If you are operating under a business name other than your personal name, you must also obtain a DBA certificate (“doing business as”) from th Town Clerk. The DBA Application must be notarized by and returned to the Town Clerk.

Business License Renewals
To apply for a Business License renewal, please fill out the following form and attach a Worker’s Compensation Insurance Affidavit as well as a completed MA DoR Revenue Enforcement and Protection Form. A check payable to the “Town of Petersham” in the amount of $15.00 must be made at least two weeks prior to the current license expiration.
